I have a round chubby face. What procedure do I need to get a more slim v shape?

PATIENT QUESTION:
​My face is round and chubby but I'm skinny. I don't like smiling because it makes my face look rounder. What procedure do I need? Buccal fat removal seems to be an option but there isn't much of a difference in before and after pictures that I have seen. I need some help
DR. MESA'S ANSWER:
Achieving a chiseled, less chubby  V shaped face requires to address multiple factors in the face and the body composition to contribute to this physical finding. This analysis will help to determine what is the plastic surgery procedure(s) what could best help to correct a chubby face, thus achieve a slimmer more chilled face. 
Some of the physical findings that may need to be addressed to achieve a slimmer / chiseled face are: 
  • Lower anterior cheek fullness: this could be addressed surgically with buccal fat removal 
  • Lower posterior cheek fullness: this could be achieving my treating the masseter muscle ( a masticatory muscle) with Botox. The masseter muscle gives bulk to the lower posterior cheek fullness at the level of the angle of the jaw bone 
  • Overall body fat: weight loss to achieve a normal or low normal weight for the height ( normal BMI) is a must. (most models with chiseled face have  are "skinny" and therefore have a BMI in the low normal range) 
  • Anterior and lateral cheek bone projection: lack of cheek bone projection won't allow to have a chiseled appearance, event if the above factors are achieved. Therefore, it someone has not good cheek bone projection, he or she may require cheek augmentation/ enhancement with either soft tissue fillers like Voluma or cheek implants 
  • Saggy facial skin: young people usually doesn't have saggy facial skin. Patients in the 30s or 40s may have some aging related sagginess that may be improved with soft tissue fillers

How to get a slimmer more chiseled face and neck?

Patient question:

I have a more round complexion to my face my lower cheeks and around my jawline make my face seem very big. I was told the cause was my buccal pad fat but after researching it it seems like it could be perioral fat and/or fat/loose skin around my jawline that offsets my face..any suggestions on what may be the best thing to do to get me a more chiseled jawline and less puffy cheek areas? Roughly what would the price range be or what would you charge? I was told around 2200 for just buccal pads..

Male patients with round face and now well defined neck could benefit from both buccal fat removal, chin liposuction ( submental liposuction) and mild upper cheek bone enhancement.

Buccal fat removal corrects the fullness of the lower anterior cheeks, thus slimming the face.
Chin liposuction, when performed all along the jaw line helps to remove the double chin and enhance the jaw line thus giving the jaw a very defined, sharp and masculine look
Cheek enhancement with soft tissue fillers like voluma, when performed with buccal fat removal, helps to achieve a V shape of the face, thus creating a chiseled look when combined with buccal fat removal.
