Hello, I have always had a round face. I am wondering what would help most to make my face more oval. Thanks Dr. Mesas Answered: Patients with round faces, secondary to chubby cheeks, that also have great prominent high cheek bones could benefit from buccal fat removal to achieve a thinner more sculpted, chiseled, v-shape facial look. Buccal fat removal is routinely performed in my practice under local anesthesia as an office outpatient procedure. The downtime is usually minimal and the results are long lasting.

Thinking there may be asymmetry? I've gotten suggestions of buccal fat removal (but worried this removes some youthful fat?) and cheek injections with Voluma (interesting but concerned this might make my face rounder?). I'd like a more longer V shape and a contoured look. less surgery is better. Are one of those two procedures my best option? Is there a way to contour my and elongate my chin/jaw line and is that even recommended in comparison? I have had a chin implant in the past as you can maybe see from the profile view. Young patients that are unhappy with the lower have of the face ( who have weak cheek bones and chubby cheeks) could potentially benefit from two Plastic surgery procedures: Buccal fat removal and cheek bone augmentation with fillers.IF in a young patient there is mild sogginess of the skin in the lower face, that is an indication that the cheek bones are weak. Therefore they should be enhanced with either fillers like voluma or cheek implants. If a young patient has chubby cheeks that get works while smiling, then buccal fat removal would address that fullness. When combined with cheek bone augmentation it helps to achieve a V shape chiseled face. I have recently been grinding my teeth a lot and notice a clicking sound of my right jaw when I yawn. I question whether I’ve developed TMJ as my cheeks feel sore. In pictures I have recently noticed my cheeks look large. Is this fat or masseter hypertrophy? what are the treatment recommendations? Dr. Mesas Answered: Fullness of the lower cheeks are caused by either enlarged buccal fat pads ( most common cause) or enlarged masseter muscles ( less common).
Chubbiness caused by enlarged buccal fat pads is located in the ANTERIOR lower cheeks. Chubbiness caused by enlarged (hypertrophic) masseter muscles is located in the POSTERIOR lower cheek. Treatment of the fillies of these tow distinct lower cheek areas is different. The enlarged anterior lower cheek is usually treated with buccal fat removal. The enlarged posterior lower cheek is usually treated with serial injections of Botox. I'M NOT SURE BUT I THINK THAT I NEED TO DO BUCCAL FAT REMOVE OP.AM I RIGHT? Patients with chubby cheeks that would like achieve a more defined face most of the time could benefit from buccal fat removal. Buccal fat removal is the plastic surgery procedure that permanently removes the fullness of the lower anterior cheeks, thus helping to achieve a more thin chiseled facial look. Very often in order to achieve a very defined face, it is necessary to have also a well defined jaw line. Therefore, in patients with not well defined jaw, specially angle of the jaw, this area could be enhanced with soft tissue fillers like Voluma to achieve a strong chiseled facial look, which greatly complements the buccal fat removal results. Buccal fat removal? I have always had chubby cheeks. In the photo I weigh 130 lbs at 5'7" it seems. O matter what I weigh, i always have these huge cheeks!! Buccal fat removal is a plastic surgery procedure that improves the fullness of the LOWER anterior cheek. Buccal fat removal does NOT improve the fullness of the UPPER cheek since in that area there is not buccal fat. Fullness of the upper cheeks could be improved with decreasing the overall percentage of body fat (%BF) with exercise and diet. Cheek bones are important aspect of a youthful facial structure. Young patients with good prominent high cheek bones look always youthful and attractive. In stead young patient with poor cheek bones look a bit aged due to sagging of the skin. Therefore, it is the structure of the cheek bones the cause of the "aging look" after a buccal fat removal procedure. If buccal fat removal is performed in a patient with poor cheek bones, the face will look a bit older since the procedure wont deliver a chiseled look (since prominent cheek bones are sciential for a chiseled young look). In my practice I always evaluate the cheek bones in patients interested in buccal fat removal. If the patient has very poor cheek bones, I won't perform the buccal fat removal if the patient does't enhance their poor cheek bones at the same time (usually with fillers). Even patients in their 50s ("not young") look younger after buccal fat removal when their cheek bones are enhanced at the same time. In conclusion, buccal fat removal won't make the face to look aged or older if the patient has good cheek bones. If the patient has poor cheek bones, he/she must enhance their cheek bones to avoid a post-surgical aged look.
